    Commonwealth Home Support Program (CHSP) providers need to submit their Wellness and Reablement Report by 31 July 2026.

    The report identifies successes, challenges and areas for improvement in delivering wellness and reablement approaches. This helps inform policy decisions and improve support for providers.

    Complete the report survey by 31 July to meet your mandatory reporting requirements.
