CHSP Provider Hub

The Department of Health, Disability and Ageing has updated the CHSP Manual.

The CHSP 2025–27 Manual provides an overview of the program, provider responsibilities, funding and program changes from 1 November 2025 under the Aged Care Act 2024. Please ensure you are using Version 5, released on their website on 30 September 2026. The Manual has 3 parts: About CHSP, Eligibility and Delivery Requirements, and Admninistration and Provider responsibilities, and 8 appendices.

Almost all chapters of the manual have been updated, along with Appendices A, F and G.

Version 5 of the manual:

• removes information relating to the transition to Support at Home
• includes information on the extension to 2029
• updates information on specialised support services, referrals, access to urgent CHSP services, the upcoming changes to end-of-life pathway, indexation payments and the glossary
• includes edits to CHSP Service List, DEX portal and reporting, home adjustments, equipments and projects, allied health and therapy, and the Modified Monash Model (MMM)
• updates information on interactions between CHSP and other projects, including Support at Home

Changes to the Appendices include:

• Appendix A - Removal of reference to AT_HM Scheme, updated service description for digital education and support, and updates to the Specialised Support Services description
• Appendix F - Version 4 updates - addition of accuracy of reporting information and My Aged Care ID reporting information updated
• Appendix G - Version 2 updates - removal of Deed of Novation requirements and added transition requirements
