  • SaH and Single Assessment System - Consultation closes 14 October 2026

    The Department of Health, Disability and Ageing is seeking service provider, aged care worker, older person and their supporters feedback on their experiences in using the Support at Home Programme and the Single Assessment System. Questions about the surveys can be directed to agedcareevaluation@nousgroup.com.

    • Aged care providers can access the survey here. (approx. 30 minutes to complete)
    • Aged care workers can access the survey here (approx. 5 minutes to complete)
    • Older people can access their survey here (approx. 20mins to complete)
    • Carers, families and supporters can access their survey here (approx. 20 minutes to complete)

    Surveys close on 14th October 2026.
