COTA Queensland CHSP Provider Update
The Aged Care Quality and Safety Commission has changed how providers can notify them about multiple associated provider changes. You told them that their current process requires multiple forms and is time consuming. In response to this feedback, they've developed a new bulk upload template option. The templates give you another way to notify them about multiple associated provider changes, in a single notification. You can still use the existing forms if you prefer.

You still need to upload the Change in circumstance base form with the bulk upload template.
If you use associated providers, you need to notify the Commission when the arrangements start, change or end. The new templates don’t change these notification requirements. Each template includes instructions to help you complete and submit your notification.
