COTA Queensland CHSP Provider Update
From 1 November 2026, all CHSP clients must have a CHSP Service Agreement in place which reflects the requirements of the Act and Rules. Previously, since 1 November 2025, all new CHSP clients needed to have a CHSP Service in place. You may create your own Service Agreement Template, or use the template provided by the Department. See the Department's guidance documentation and template here.
The CHSP Service Agreement reflects the following requirements:
You are required to make the Service Agreement readable and understandable. Your clients must be able to understand their rights when accessing funded aged care services. Version 2 of the CHSP Service Agreement User Guide is now available.
From 1 November 2026 all CHSP clients approved to access funded aged care services must have a Care and Services Plan in place which reflects the requirements of the Aged Care Act 2024 and the Aged Care Rules 2025 and includes:
