The Department has released the CHSP 2025-2027 Manual.
The CHSP 2025-27 Manual and Appendices are now available on the Department of Health, Disability and Ageing’s website.
Key program changes to the Manual include:
Collection of My Aged Care IDs – providers will need to collect their clients’ MAC IDs in their own client management systems in preparation for this functionality in DEX from January 2026 (see Chapter 12).
Client assessments – all clients wanting to access subsidised CHSP services need to have an aged care assessment and be deemed eligible to receive those services. This is an existing program requirement and will prepare providers and clients for the Aged Care Act commencement on 1 November 2025 (see Chapter 5).
Modified Monash Model 5 (small rural towns) up to 20% loading (see Chapter 7).
Home adjustments – the Commonwealth contribution has been increased from $10,000 to $15,000 and is available per client per financial year (see Chapter 7).
Client contributions towards Allied health clinical services – client contributions will continue for these clinical services (see Chapter 7).
Provider relinquishments – notice period remains as 5 months and new exit dates of 31 December or 30 June are detailed in Chapter 9 and the new Selections Framework at Appendix G.
Flexibility provisions continue, with additional services needing written approval from the department (see Chapter 8).
Child Safety Annual Statement of Compliance – new Commonwealth grant agreement requirement (see Chapter 12).
Financial declaration with new statement of compliance that Commonwealth funding has been spent on assessed clients (see Chapter 12).
The Manual Appendices include the following:
Appendix B – Embedding Wellness and Reablement in the CHSP Overview (minor editorial changes)
Appendix C – CHSP contacts, support and resources for providers and clients (no changes to existing content)
Appendix D – CHSP Police Certificate Guidelines (no changes - same as current appendix)
Appendix E – CHSP National Unit Price Ranges and National Guide to Client Contributions Framework (minor updates to unit pricing table and MMM)
Appendix F – CHSP Compliance Framework (updated to reflect 2025-27)
Appendix G – CHSP Selections Framework (new appendix)
The Summary of changes document provides a high level summary of all changes in the Manual and is published alongside the Manual and Appendices.
The new Service List Mapping guidance document is also now available to support providers with the staged transition of service lists from 1 July 2025. The CHSP Service Catalogue has also been updated.
