COTA Queensland CHSP Provider Update
Fellow providers Sector Support and Development Central Coast & Hunter are hosting an Integrated Assessment Tool Webinar
From 1 July 2024, the Integrated Assessment Tool (IAT) replaced the National Aged Care Screening and Assessment Form (NSAF) and has become the way older people are assessed for eligibility for government-subsidised aged care. Understanding this change is fundamental in preparing your home care service for future reforms.
The change to the IAT will allow assessors the opportunity to collect more complete information about the older person and ensure service recommendations and referrals are tailored to their current needs. It will also allow providers to align their current assessments to the new tool.
The use of recognised assessment tools is essential because older adults often present diverse and intricate care requirements across various domains of health and well-being. These tools help in
systematically evaluating factors such as physical health, mental well-being, functional abilities, social interactions, and spiritual needs.
The 2-hour training session, delivered online will cover:
�. Understanding the purpose and benefits of validated assessment tools for CHSP providers.
�. Familiarising participants with different types of assessment tools and their specific applications.
�. Learning how to select the appropriate tool for assessing various care needs.
�. Gaining practical skills in using the Integrated Assessment Tool (IAT) for tailored assessments.
�. Applying clinical knowledge effectively during assessments to enhance accuracy.
�. Interpreting assessment outcomes to develop personalised care plans.
�. Aligning current assessment practices with the new Integrated Assessment Tool.
�. Preparing CHSP providers for future aged care reforms by integrating the IAT.
